Hoosier Healthwise
Hoosier Healthwise (HHW) is Indiana’s Medicaid program for children under 19, pregnant women, and some families. It covers a wide range of health services at little or no cost.
Who It Covers
- Children ages 1–18 in families earning up to 163% FPL
- Infants under age 1 and pregnant individuals earning up to 208% FPL
- Children via CHIP Package C — families earning too much for standard Medicaid but up to ~255% FPL; small monthly premiums may apply
- Parents and caregivers in very low-income households with children
- Former foster youth who were in Indiana care at age 18 qualify through age 26 with no income limit
- Newborns — automatically enrolled for one year if the mother has Medicaid
Children in families with the lowest incomes pay nothing. Families with slightly higher incomes pay a small monthly premium (usually $1–5 per child).

Choosing a Health Plan
Managed Care Organizations (MCOs)
Once enrolled, your child chooses — or is assigned — one of Indiana’s Medicaid health plans (effective January 1, 2026):
- Anthem (Healthy Indiana)
- CareSource
- MHS (Managed Health Services)
You can switch plans within 90 days of enrollment or once per year during open enrollment (November 1 – December 15).
